LISA Applications
Below you will find two types of membership applications: a General Application and a Law Firm or Accounting Firm Application. Please download and complete the appropriate application. When complete, mail the application, along with all requested supporting documentation and a $500.00 check made payable to the LISA, to:

IMPORTANT NOTE FOR PROVISIONAL MEMBERSHIP APPLICANTS
When applying for Provisional Membership, an Income Affidavit Form must be completed and submitted with the General Application. Download the Income Affidavit Form here. To learn more about Provisional Membership, download the Benefits of Membership Guide here. Finally, The application review process generally takes two to three weeks. The LISA Board meets at least once per month to consider applications.
